Patients, their families, and caregivers are often ill-equipped to discuss and make choices related to life-limiting illnesses. They may be unaware of the choices surrounding advanced illnesses, including documenting one’s final treatment preferences and the benefits of hospice care.
Votiva’s staff specializes in addressing the difficult topic of advanced illness planning and works longitudinally with patients, their families and physicians. Our clinical staff work directly with patients to discuss their health history and health care options for the future.
We take the time to build relationships and earn the trust of patients and their families. To us, this work is a calling. We offer emotional support in time of need and specific information to help them understand all of the things they are entitled to receive. We teach and answer questions and help lead them to the path of most support.
We offer further support to patients and their families with print and online education and resources for decision-making and appropriate advanced illness choices including use of our affiliated website, My Care Community. Through www.mycarecommunity.org, we provide patients, families and caregivers (informal, secondary, or family caregivers) much needed support and information. The site provides the necessary resources to help caregivers and families face chronic disease and the latter stages of life with dignity and comfort.
